Event Information Form
Prior to submitting this form, make sure you have reserved a venue with the appropriate department.
This form is used to request physical plant, media services, dining services, public safety, and publicity needs for your on-campus program. This form will be submitted to the departments listed above and a representative from that department will contact you with further information regarding your request.
All Shaw Living Room space and The Den should be reserved with Student Activities at ext. 4763 or 4634.
Shaw Servery Room should be reserved with Aramark at ext. 4584.
The Blue Room, Carlson Conference Room, Carlson Auditorium, Carlson Great Room, Vans, Smith Conference Room and Phaneuf Conference Room should be reserved with Phyllis Persky at ext 4519.
The Continuing Education Conference Room should be reserved with Bee Magliozzi at ext. 4040.
All Athletic Facilities should be reserved with Athletic Department at ext. 7211.
All Classroom space should be reserved with Registrar's Office at ext. 4503.
Please note that when you request banner space, this is just to reserve the space for which you must make your own banner using the materials available to registered groups in Shaw 3.
Also, as many services require some advanced notice to prepare, please submit this form no later than 1 WEEK PRIOR to your scheduled event. This way all the departments can do our best to accommodate your requests for a successful event!
